The most common diagnoses are cancer at various stages, multiple chronic conditions, COPD, and heart failure.\nPatients may be in curative, palliative, or terminal care phases. Some patients end their lives at home. We also admit patients with short care periods, such as those undergoing infection treatment.\nPatients are primarily referred from major hospitals in Stockholm but also from primary care.
