In our smaller work group, there is also the opportunity to develop in areas that you are personally interested in.\n\n1 position(s).\n\nJOB DUTIES\nAs a case manager within elderly care, you investigate the need for services and make decisions according to the Social Services Act. The assignment includes, among other things, advisory services, planning, coordination, and monitoring of interventions.\n\nYou are responsible for a geographical area where you handle cases such as home care, respite care, short-term care, safety alarms, and special housing.\n\nAn important part of the work is planning related to discharge from the hospital. One case manager at the unit primarily works with discharge processes, while other case managers mainly work with cases in ordinary housing and short-term care.\n\nIn Hallstahammar Municipality, we value collaboration, which means that your work involves many contacts with different professional categories. You will also have the opportunity to contribute to the unit's development work by bringing forward ideas or leading your own side projects.\n\nQUALIFICATIONS\nWe are seeking someone with a degree in social work or another degree that the employer considers equivalent. Knowledge of social law is important for this position.\n\nIt is meritorious if you have experience in public administration, particularly within healthcare and social services. Previous experience in another role within elderly care is also meritorious.
