An important part of the role is to work preventively and promote health, where continuity in patient contact provides the opportunity to observe changes over time and adapt support according to patient needs. You work independently in your clinic, but with colleagues nearby when you need to discuss or gain additional perspectives. Together with physicians, nurses, care coordinators, rehabilitation staff, and the psychosocial team, you contribute the district nurse perspective in the work around the patient.
