The position involves working across the entire municipality and enables safe and secure transitions home from inpatient care.\nYour work includes conducting activity assessments and training, testing and prescribing physical and cognitive aids, and writing certificates for home modifications.\nYou will mentor and train nursing care staff and supervise students.
