The position is full-time with daytime work on weekdays.\n\nYour role in the team\nAs a nurse with us, you will follow patients over time and serve as the responsible nurse, PAS, for your own patient group within your area. You will conduct clinical assessments, plan interventions, and follow up on care as needs change. An important part of the work is also to strengthen the patient's own resources and create conditions for participation and independence. You will become part of a multidisciplinary team where nurses, physiotherapists, occupational therapists, and physicians work closely together. We have fixed physician contacts through our health centers, recurring rounds, and multidisciplinary meetings with home rehabilitation. Together we coordinate interventions, share knowledge, and support each other when patient needs change.
