We have joint case reviews\/meetings 2 times per week. As a care coordinator, you work in LINKEN to coordinate contacts and plan the interventions needed for the patient regardless of age, diagnosis, living arrangement, and who have a need for coordinated planning. This may mean that a care plan, SIP, team meetings, etc. are needed. There are opportunities here to develop the primary care center's working methods and routines to early identify patients with coordination needs. You are also a support for colleagues in this process.\n\nYou will also work with reception services, assessment consultations, and telephone advice. We work in a multiprofessional team and apply work with fixed physician contact\/fixed care contact.
